So as you know, I purchased the CL simple 70 in a size 4.5 as well. BUMMER!! Although, my heels did not slip out in the back, the size 4.5 was extremely tight in the front. My toes also touched the front side of the shoes and was not comfortable. Since it is patent leather, I don't expect it to stretch out as much as it would as regular leather.
I really wanted to make the CL shoes work.... Unfortunately, I have to return the size 4.5. I also returned the size 5 because it was too big even with foot cushions. I must be a size 4.75. Who sells that!?

Heels Please! VANELi and Christian Louboutin
I've ALWAYS wanted a pair of pumps that will fit my tiny and narrow feet! I gave up searching for a while because nothing worked (steven madden, aldo, cole hann...list goes on). I must have been wearing the wrong size shoes since high school because since then my shoe size is going down. I used to think i can fit a size 6. Boy was I wrong! I'm about a size 5. Maybe even a 4.5???? Sometimes I can wear 5.5 in winter boots.
I ordered a few pairs of pumps but ultimately I decided to keep the Vaneli Undine Pumps in Black Leather size 5 because these fit me best. I have also wanted a pair of Christian Louboutin Simple 70 Nude Patent so bought that as well in size 35.
Here are some photos to compare the similarities of the two:

They almost look exactly the same in terms of shape and heel height.
Here are the differences:
1. CL shoes runs more narrow in the FRONT. My toes get squished.
2. CL shoes runs a little more longer? compared to the Vaneli. Heel slippage! I ordered a 34.5 also to see if it fits better (still waiting for the shipment).
3. CL shoes have a very slippery sole but that is no surprise.
With any type of shoes, I usually have to make some kind of adjustments on my own to improve comfort and fit:
1. Narrow heels:
Initially, when I first tried on the Vaneli pumps, my narrow heels slid out of the shoes a little. I figured I can just add some foot cushion and solve that problem since it wasn't so severe. (I'm not a big fan of heel liners. I once bought a pair of gel heel liners and the liners decided to come off while I was walking instead of doing its job.) Since the weather has been really warm lately, my feet are somewhat swollen so I haven't had the need to put the foot cushions in. In fact, when I put the foot cushions in, the front of my feet were felt too tight in the shoes. I might put some mole skin on just the sides of the heels to help it a little. I haven't tried yet but it's an idea.
2. Big toe and the little toe blisters: The sides of those toes rubs on the inside of the shoes and creates pain! I put some mole skin padding where my big toe and little toe would rub in the inside to make them more comfortable. (You can kind of see it in the first photo).
3. Super slippery soles: Why don't all shoe companies just put some rubber on the soles so we don't fall and make a fool out of ourselves? I guess life isn't that simple. I bought some Sole Stopperz and similar ones from DSW. Haven't tried them yet but I hope they work. If they don't work another way I try to create friction is to scratch the bottom of your shoes on some rocks outside. I know it messes up the soles but it does make things a little better...... I think they secretly want to see us fall.
I already walked in the Vaneli pumps and they are really comfortable! I haven't decided to keep the CL shoes yet. I have to wait for the smaller size to see which is better.
